Supporting Stroke Recovery at Home: SAFE and PEER-HOMEcare

Stroke is a leading cause of long-term disability worldwide, with millions of new cases each year and many more people living with its ongoing effects. As survival rates improve, the need for effective long-term rehabilitation and support continues to grow. However, access to rehabilitation after hospital discharge remains inconsistent across Europe, contributing to unequal care, fragmented services, and gaps in recovery support.

These challenges are compounded by workforce shortages, limited community rehabilitation services, and variation in how post-stroke care is funded and delivered. As a result, many stroke survivors face significant barriers when transitioning from hospital to home.

The Stroke Action Plan for Europe (SAFE) 2030, developed in collaboration with the European Stroke Organisation (ESO) and the Stroke Alliance for Europe, highlights the urgent need to strengthen post-stroke care. A key priority is improving continuity of rehabilitation and expanding innovative approaches that support recovery beyond the hospital setting. SAFE also recognises the home environment as a central setting for high-quality, person-centred rehabilitation.

Aligned with this vision, PEER-HOMEcare aims to support stroke recovery where it matters most...at home. The program focuses on enabling stroke survivors to engage in meaningful daily activities within their own environment, supported by therapists, family members, and caregivers. Rather than relying solely on clinic-based rehabilitation, PEER-HOMEcare integrates recovery into everyday life, helping individuals rebuild function, confidence, and participation through real-world practice.

By combining structured support with home-based rehabilitation tools and collaborative care, PEER-HOMEcare reflects the direction set out in SAFE 2030: improving access, continuity, and equity in stroke recovery. It recognises that recovery does not end at discharge from hospital—it continues in the home, where life is lived.
