What is PEER-HOMEcare?
PEER-HOMEcare is a European Transforming Health and Care Systems (THCS) funded project, aiming to support stroke rehabilitation through the utilisation of the home envrionment.
This novel approach creates a stimulating home environment that invites stroke survivors to perform frequent and varied movements with the affected limb throughout the day.
Participants take on an active role in shaping their recovery at home, working closely with a therapist to integrate everyday routines as part of the recovery process.
Using personalised strategies, daily routines, and practical changes within the home environment, PEER-HOMEcare aims to build confidence, independence, and meaningful progress over time.
Families and caregivers are included as important partners in the rehabilitation process, helping create a supportive, motivating and enriched home environment.
The project is currently in testing and piloting its feasibility across Norway, Sweden, and Latvia.

Explaining the PEER in PEER-HOME-care
Our approach is based on understanding how the body and brain move and learn. We work closely with stroke survivors, health professionals, and other stakeholders, regularly reflecting together on practice.
Through collaboration, communication, and shared learning, we support confidence, independence, and continual improvement for all involved.

We focus on creating a stimulating and supportive home environment that encourages participation, motivation, and ongoing recovery.
By adapting everyday spaces and routines, we help bridge the gap between clinical rehabilitation and real-world living, making rehabilitation meaningful and effective over time.

Rehabilitation is personalised to each individual, focusing on meaningful goals and evidence-based strategies that continue beyond hospital discharge.
Working together with therapists, families, and caregivers, we support long-term recovery, greater independence, and improved quality of life through everyday activities and routines.
