The home is more than a living space - it is a powerful setting for daily repetition, real-world task-practice and rebuilding autonomy.

Our research focuses on harnessing the home environment to support meaningful, high-frequency practice that promotes independence after stroke. Built through a translational research pathway, from theory and co-design to real-world testing and feasibility evaluation, our work is grounded in evidence and shaped by the needs of stroke survivors and clinicians.

Transforming Health and Care Systems (THCS)

PEER-HOMEcare is funded by the Transforming Health and Care Systems (THCS) partnership. THCS support has enabled the co-design, development, and evaluation of the PEER-HOMEcare program, helping progress the project to a feasibility trial.

The THCS partnership also provides ongoing oversight and guidance through structured reporting processes, including mid-term progress reporting, ensuring the project remains aligned with its aims, milestones, and wider system-level priorities. This support has been instrumental in strengthening collaboration across partners and translating the PEER-HOMEcare approach into a tested and implementable model of home-based stroke rehabilitation.
